Unless otherwise prescribed, 1-3 squirts at about 30
seconds intervals are sprayed into the mouth during
an attack or shortly before physical and/or mental
stress that may trigger an attack. The single dose
of 3 squirts applied to curtail a painful attack
should only be increased upon the physician's
express advice.
In acute myocardial infarction or acute heart
failure one starts with 1-3 squirts. In the event of
non-response within 5 minutes, an additional squirt
may be given. In the case there is no improvement
within the subsequent 10 minutes, spray application
may be repeated under close blood pressure
monitoring.
For prophylactic treatment of catheter-induced
coronary artery spasms during diagnostic or
therapeutic surgery, 1-2 squirts are given directly
prior to catheter handling. If required, spray
application may be repeated under constant
circulatory control.
The solution is to be sprayed into the mouth; it
should not be inhaled. |

Vaso-dilated nitrate headache may occur frequently.
Flush, dizziness, orthostatic hypotension and reflex
tachycardia have been documented in some instance. |

-Hypersensitivity against nitrates or one of the
spray ingredients.
-Acute circulatory failure
-Marked hypotension
-Cardiogenic shock, unless a sufficiently high left
ventricular enddiastolic pressure is maintained by
intraaortic counterpulsation or positive inotropic
drugs.
-Hypertrophic obstructive cardiomyopathy,
constrictive pericarditis and pericardial effusion. |

The label of
the spray bottle bears an arrow mark at its
lower margin. As soon as the fluid level in the
bottle reaches this points, a new package of
isoket spray should be at hand for safety
reasons.
Do not store at temperature below 10℃!
The solution contains 89.6 vol.% of alcohol. |
